Hourly Nurse Case Manager to handle Workers Compensation cases in Northern Virginia and D.C.. Position researches and recommends resources and creates flexible, cost-effective options for catastrophically or chronically ill or injured individuals on a case by case basis to facilitate quality individualized treatment goals, including timely return to work when appropriate. Laptop, iPad, phones are provided. Full benefits available when CM consistently works 30+ hours per week. Position allows working from home and is driven by deadlines, case urgency and specific account instructions.
Provides medic! al case management to individuals through coordination with th! e patient, the physician, other health care providers, the employer, and the referral source.
Coordinates Independent Medical Evaluations. Provides assessment, planning, implementation, and evaluation of patient's progress. Evaluates patient's treatment plan for appropriateness, medical necessity, and cost effectiveness. Implements care such as negotiating and coordinating the delivery of durable medical equipment and nursing services. Devises cost-effective strategies for medical care. Attends doctor's attorney's visits. Attends hospital and/or long-term facility discharge planning conferences. Assesses rehabilitation facilities for appropriateness.
Performs or coordinates architectural assessment of patient's home. Required to read extensively. Required to prepare organized reports within a specified timeframe. Required to use telephone extensively. Required to research medical and community resources for patients with catastrophic or chronic diagnoses. Heavy loca! l travel required. Minimum Billable Standards are 1.8 to 2.2 hours per case, per week. Case load may get up to 25 cases at a time. Requires regular and consistent attendance. Additional duties as required.
Graduate of accredited school of nursing. Current RN licensure VA required. Licensure in DC desirable. Three or more years of recent clinical experience, preferably in rehabilitation or home health. Case Management preferred, but not required. Must be computer literate and be able to type case manager reports.
