Case Manager

The Case Manager supports a caseload of 12 - 29 households fleeing or attempting to flee domestic / sexual violence or human trafficking to obtain and maintain safe housing in the Artemis House Transitional Housing or Domestic Violence Bridging Affordability programs. The Case Manager will asse ss victim / survivor needs, connect to appropriate services, and advocate to ensure the provision of comprehensive services which meet the identified needs. The Case Manager will implement services with a trauma-informed, strengths-based and client-centered approach while assisting households to work toward self-sufficiency. The Case Manager will work closely with the Artemis House Senior Housing Specialist to accomplish program goals and priorities in accordance with agency philosophy and values, and best practices for homelessness and survivors of domestic violence.
Salary Range: $40,000-44,000/year
Duties and Responsibilities:
Provide crisis intervention services and develop individualized safety plans for each household in the Artemis House Transitional Housing (5 units) and Domestic Violence Bridging Affordability programs (up to 24 units)
Perform assessment of clientâs physical and mental health, social & life skills needs , employment history, educational needs, safety, strengths and other relevant areas to determine, in conjunction with the client, what supports are needed to develop and achieve housing and safety goals
Collaborate with clients to develop housing and safety-focused service plans individualized to each clientâs unique needs
Meet with assigned clients regularly and provide intensive case management and advocacy services to support clients in achieving their identified goals
Assist clients in filing for protective orders and completing affidavits; accompany clients to court proceedings and various appointments (i.e. attorney meetings, protective order hearings, custody hearings, and housing interviews) as needed
Coordinate services amongst mental health service workers, service providers, advocates, the legal system and other involved parties to ensure effective service provision
Provide housing search assistance to clients; suppor t clients in developing and maintaining a budget
Communicate and establish working relationships with landlords to negotiate lease terms and advocate on behalf of clients
Disseminate information related to tenant rights and responsibilities, housing discrimination, communicating with landlords, basic unit maintenance, lease compliance, and housing search and location techniques
Coordinate the provision of rental assistance, track all payments provided and payments received
Maintain current knowledge of issues impacting individuals experiencing homelessness and domestic violence
Work with the Senior Housing Specialist to ensure that services are implemented in accordance with agency philosophy, best practices, and all applicable federal, state and local regulations
Document all contacts with clients in client file and databases as required; maintain complete client records in accordance with agency and best practice standa rds
Adhere to Bridging Affordability guidelines and all applicable contract and funding regulations
Assist Artemis House hotline callers with crisis intervention, safety planning, and service coordination as needed. Serve as a member of the Artemis House team.
Participate in staff meetings and trainings as directed by supervisor.

Qualifications:
Required:
A bachelor's degree in human services/ related field or commensurate experience
1+ years of case management/ direct services
Strong written and oral communication skills
Ability to prioritize competing priorities and make sound judgements
Ability to complete tasks while navigating frequent interruptions
Ability to deliver creative, resourceful solutions to unique challenges
S trong organizational and time management skills
Valid Driverâs License and reliable transportation
Ability to drive a minivan or 13 passenger van
Willingness to work evenings and weekends when needed
Preferred:
Experience with providing services to clients experiencing homelessness, victims of domestic violence / sexual violence, mental health or substance abuse issues
Foreign Language fluency in a language other than English (ex. Spanish, Farsi, Arabic or Amharic)
Physical Requirements:
Annual TB Test is required
Ability to sit or stand for long periods
Ability to lift items weighing 10-20 pounds
